In fact, i want to apply the PR sponsorship (sub class 886) previosly,
however, i get the result today and fail to get all six in four IELTS components (6.0 6.5 6.0 5.5).
I am wondering i can apply the temporary visa (sub class 485) or not?
Even i try to apply visa 485, i find that i cannot choose the right english ability level (only proficient and compentent can choose). What can i do?
In addition, is that i need to take the medical check immediately?
Since my visa expired in the end of this month, i am in a hurry.

No, you can't,
You have to get all 6s in IELTS to get a point 15. Otherwise your point won't satisfy "at least 110" requirement.
In this case, you don't need medical check until you get all 6s in IELTS.
In order to get a 485, the only thing you can do is IELTS retest.
